About Financing
We accept cash, personal checks, and all major credit cards.
For your convenience we will file all insurance claims and we are providers on many plans. Our staff will gladly assist with insurance verification, pre-certification and/or pre-determination of benefit needs.
Your medical and/or dental insurance coverage is designed to share in your healthcare costs; the patient or guarantor is fully responsible for complete payment. Please remember you are fully responsible for all fees assessed by this office regardless of insurance coverage.
We have made arrangements to have an easy financing application procedure should you prefer outside financing assistance.
Elective and cosmetic procedures require a 25 percent deposit at the time of scheduling. Complete payment must be made at least one week prior to scheduled treatment time. Cancellations must be made at least 48 hours prior to the scheduled appointment or the deposit will be forfeited.
To help us in assisting you, we ask that you keep us informed of any change to your insurance. It is important that all personal and insurance information be kept current. |
